Apologies for the delay in getting back to you, and thanks for your patience.
Yes, you can add the register form on any page you wish. I don't think there's support for an avatar off the top of my head.
This add-on only works with C5.6.x. In order to make it work with 5.7+ it has to be re-built from the ground up. Currently I'm only doing that on a custom project basis. For this type of add-on the cost would be several hundred dollars. If you're interested just let me know.
