Express form will not submit if testimonials block is on page

We have an express contact form and testimonials block on the home page of web site.
When submitting the form it goes to a blank white screen. No Error codes.
If the testimonials block is removed the form works as expected.

This has now become more of an issue as it has been noted that when the express form is submitted it is adding a blank testimonial as well.

I have ran a few tests on a couple of other Dev sites and it is also occurring on them so it is not theme related or unique to just one site..
