Depends on which version of Concrete5 you're using. If 5.5, it's under Dashboard -> Blocks, if 5.4 it's under Dashboard -> Add Functionality. Either way, when you get there, click the "Edit" button next to the block, then click the "Uninstall" button (or maybe it's called "Remove"?).
