Extended FAQ block type not showing

We purchased the extended faq add-on in July 2013 and gave up on trying to use it. We are trying again as it looks like a fantastic add-on.
We have access through the back-end to add categories and entries but the block type does not show anywhere.
Suggestions?

I am trying Extended FAQ again... because I really really want it to work!
I uninstalled it, reinstalled it -- and get this error message:
mysql error: [1048: Column 'btDisplayOrder' cannot be null] in
EXECUTE("INSERT INTO
BlockTypes(btID,btHandle,btName,btDescription,btActiveWhenAdded,btCopyWhenPropagate,btIncludeAll,btIsInternal,btInterfaceWidth,btInterfaceHeight,pkgID,btDisplayOrder)
VALUES (NULL,'extended_faq','Extended FAQ','Adds a Extended FAQ section to
you pages.',1,0,0,0,400,300,'23',NULL)")

Not sure what is going on here...
The error you are getting is one of C5 itself and not from Extended FAQ. Check the version you are trying to install. Is this up-to-date?
