div tags not closed? errors for end user unable to fill out form.

ISSUE:
using formidable to have a client submit media orders on our website. client has been getting fatal errors when submitting properly filled out form. (first attachment)

my programmer investigated, updated the add on

NOTES FROM MY PROGRAMMER:
noticed there was an update to the add on and installed. The update did complete and I was able to successfully submit the form without any errors when not logged into the website and using Explorer.

mysqlt error: [1075: Incorrect table definition; there can be only one auto column and it must be defined as a key] in EXECUTE("ALTER TABLE FormidableSavedSearches MODIFY COLUMN searchID INTEGER(10) UNSIGNED NOT NULL AUTO_INCREMENT")

There are some issues with the updated version however. It looks like the developer didn’t close div tags for some of the titles and as a result the source code is showing.


NEW ISSUES:
as of this morning, my client is having new issues (from the client):

I am having an issue with the order form this morning. I have tried to use it on multiple browsers (Firefox, IE, Chrome) to no avail.

Issues:
· boxes for Rep Name, Start Date/End Date is missing.
· button for Client Provided Ad materials is missing.
· Dropdown lists do not populate information, display a blue line only

FINAL:
we've rebuilt the required fields into the default C5 form, but it doesn't do all we needed, and any orders submitted will not be saved in the same area as formidable, meaning we cannot download a history of insertion requests from one location, increasing the chance of errors/ missed orders/missed billing. Really want to figure out why this is occurring and fix it quickly so avoid further disruption to this client's interaction with our website.

The error on update is a known one. It happens when a older version (before 2.0.10) is updated to 2.0.15. The error isn't nice, but doesn't break the update process. So there is no problem there so far.

The issues mentioned in the screenshots is because of the older version your have been updating from. The template you are using (probably a custom one) needs to be updated. You can test this by choosing a default template for Formidable.

Then the error on line 142. I think you need to clear the cache of the website. If that doesn't work you need to check if you have custom "formidable.php" in the /root/models directory. The error is a known issue from a older version.
