Have you modified screen.css at all? I've attached a new version, so if you haven't made any changes yet you could just upload that to /packages/themes/street/

If you've modified it, you can just add the following code:
At the end of the file before the } just add: .site form table tr td {display: block}
Also, around line 250 just after .site div#footer-nav ul {display: none}, add this: .site form table tr td {display: block}
