Form Submission email content fields blank, but Title fields fine

The form submission works but the emails we receive are blank where user input is. The Titles of the fields show up fine and the entire form, user content included, is all there in the dashboard... just not in the email notification. We are using the standard C5 form and the following is the environment:

I haven't taken a look but I haven't modified the form file as the response asks. Do me a favor and comment on the other post so we can keep it all in one place.
