The client wants the ability to create columns within the form and wants the user to be able to add additional fields if they need to.
Does anyone know if this add on or any other add on will do this?

I'm having trouble getting to this page, but no Advanced Forms does not allow the user to add fields to the form.
Do you know if there is anything out there that does offer this?
