Advanced Forms add-on (EDIT) on display list has issue with images

Has anyone experienced a problem with updating images in the 'EDIT RECORD' form component on a data display list block.
In more detail, the text fields in the 'EDIT RECORD' mode DO update fine. However, Images will not clear when [x] is clicked. Similarly,
when clicking on the image link it will open the file manager where you can CHOOSE another image---but this returns to the 'EDIT RECORD' form without making any changes whatsoever. There are no errors
