Field answer to subject line, fields in notifications and multiple checkboxes.

Hey,
Thanks for this add-on; it's great! A few questions though:

1) Is there a way to add in a field answer to the subject line of the notification email? So, if one of the fields was "date requested" then I could put the answer to that field in the email subject?

2) Is there a way to include the form answers in the notification email, below the custom message?

3) Instead of having a multiple select box, can I have multiple checkboxes?

And, for the third question, I don't think that I was clear. I would like to be able to have multiple checkboxes act like a multiple select box. So, one question with many options that generated multiple checkboxes. Is that possible?

To make sure, by "notification email" do you mean the email that goes to the site admin, or the email that goes to the user?

By notification email I mean the one that the site user gets after submitting the form. We want them to get a copy of their submission under the custom message.

I would also be very interested in having the ability to have a field from the form supply the value to be used as the subject line of email notifications generated from the form. Both the email from name and email from address have this as an option, and allowing the subject line to also be configurable would be a useful addition.

We are using advanced forms with our concrete5 installation, and are generally happy with how it works. But having this as an option, in addition to the current functionality of a fixed subject line for all messages, would be very helpful.
