No Data Display Templates

Permalink Browser Info Environment
In the form notifications no data display templates are being loaded when I check the box: Use Data Display Template. I'm seeing this error.
[07-Jul-2015 11:14:32 America/Chicago] PHP Warning: Invalid argument supplied for foreach() in /public_html/packages/sixeightforms/tools/form/notifications.php on line 151[\code]
I created a template in data display but it's not visible. We just recently updated both Advanced forms and Data Display. Before the email notification text box has something like this in it:
[code]
Thank you for submitting a request. The information you submitted is currently being processed:
<br />
<br />
<b>Your Name</b>
<br />
<field name="Your Name" />
<br/>
<br />
<b>Your Phone Number</b>
<br />
<field name="Your Phone Number" />
<br/>

The email does not replace any of those fields, and does not send as HTML. I erased it and tried to add the Data Display Template to see if it would fix the problem.
