I would like to send an e-mail notification for a submitted form to a specific internal e-mail address based on a bit of data passed to the form. In particular, a job description page will contain a link to an advanced form, and based on the job ID (or some other attribute) I would like to direct the e-mail notification to a particular e-mail address.

My goal is to have a generic job application form. Of course, one possibility is to create N copies of the form and just change the e-mail notification address. But, I prefer to avoid that due to the maintenance overhead.

I see that custom notifications can be created based on form fields. But, what are possible methods for passing the form this bit of data to be used to make the e-mail notification decision?
