Could you uninstall the package and after that check if the page appears in your sitemap. If it does delete the page, including its parent, and install the package again.

Let me know if that doesn't work.

Oh wait, I just checked your environment information and there's a problem.

You are using a version of the package that will not work for concrete v9. You have to download the latest version which is 2.0 and it will only install on concrete 9.0.1 and above and you're using 9.0.0

So you have to update concrete and then download version 2.0 of the package and try again.

upgraded to 9.0.1 the extensions shows v 2.0, yet no change after reinstall... no errors but it's not showing in the dashboard:

that's not even technically possible as the site is on a private network and there is no cPanel. I can provide as much logs as necessary ro do any operations you might recommend.

Ok so let's try this. First uninstall the package completely.
Then go to your database and look for the following entries. If you find them delete them.

Table Pages:
Look in column cFilename for values /dashboard/ajaxexpressforms/view.php and /dashboard/ajaxexpressforms/shared.php

Table PagePaths
Look in column cPath for values /dashboard/ajaxexpressforms/shared and /dashboard/ajaxexpressforms

Table Collections
Look in column cHandle for values ajaxexpressforms and shared

If you find any of these values delete the row.

Once you're done install the package again.
