Getting data to display

I know that it is just something simple that I am missing but I can not get any of my data to display. I have a form that collects the data. A template shows up on the new page, it looks like it makes room for the data and nothing.
What am I missing? Please help!

I deleted everything, started over and it seems to be working. I am adding one element at a time to see if I can determine what I did wrong.
