I would like an email on each record that I can put in the template going to a dedicated email address. But it needs to pick up data from Advanced Forms also and embed that in subject matter of the email.

So I'd like a link/button called 'Request' that via php would send an email to info@ and then within the content of the email I would the info below pulled in if poss from AF.

I would like the details of John Smith (name entry in AF) dated 05/11/1992 (Date entry in AF). Comments - Store No. A010 (Comments entry in AF). Then some extra sign off text.

Is this possible? If so could you help me with this bit of code please...? Many thanks in advance
