Using field data as URL

I've got a form with several address fields. Is there a way to use the text from those fields as part of a URL? For instance, I'd like to add a Google Map link, using several fields, such as:

http://maps.google.com/maps?q=[Address1]+[Address2]+[City]+[State]+...

Is is possible to hard-code something like this into a template?
