Accept Terms of Use

Is it possible using the Extended form to allow the user to indicate if they "accept the user agreement" by clicking a check box for "Yes" or "No"?

E.g. Downloading any new software off the web the user needs to indicate "yes" or "no" that they agree with terms, conditions etc.

The key is to keep it in the form so that we can track responses.

I tried setting using the standard form by entering the Terms and Conditions in the Question field and going with a "Check box" list, however, the Terms and Conditions are too long.

Does any one have any suggestions.

Hi James,

You could set a checkbox that the agree.
In the settings of the form you can add an attachment to mail when the click send.

So if they accept the software or PDf will be mailed. And the info is registered in the dashboard.

This should be possible!
