Is is possible with the Extended Form add-on to style a survey so it would look like this:

Basically it needs the same answer categories for each question. Also each section of the form needs to be submitted as the participant moves to the next page.

It should be possible.
All the fields are in div's so just change the stylesheet of the block.
You could also add a template with the correct style.
If you are able to make it, it would be nice to send it to me, so I can add it to the addon.
