form view is akward, htaccess issues? php? java?

i am not sure, maybe this has something to do with htaccess, java or php issues? created the form just fine, but when i put it on the page it doesn't look right. i un-and reinstalled it but got the same results. cleared caches. same results. prior to the test uninstall i tried to submit the form anyway, but it just got stuck and finally kicked back a 500 internal server error. had to wait several minutes for the site to come back online. your thoughts would be really appreciated.

Hi,

This is your theme. The styles of the theme are overwriting the defaults of the form.
Can't help you there. You could try to contact the theme builder.

yes, you are correct. i checked with the theme designer (Venture) and it turns out that there could be a problem if the Forms Add on is using the same class names in their markup as the foundation framework- (which Venture is...). Since I am not familiar with steps needed to change the class names, I should be looking for a Forms add on that uses a different framework. I am a bit disappointed because i really liked the add on, but I learned something new about matching add-ons with themes....
