htmlspecialchars() errors

i made form which user can send pictures to our e-mail.
i test it with pictures(10kb~10mb),but the server always sends warning like this:

and no email attachment(even checked option),and 0kb file in filemanager made.
how can i fix it? or i do something wrong?

i think formidable cant upload files to filemanager.
i check filemanager but no file from formidable found.

i also found 0kb file in uploading page at beginning.here is image.
