I have posted the 2 previous questions with no response. We desperately need help as your add-on has a few bugs that we are unable to figure out. In addition to my 2 previous questions, we are now having an issue with the "required asterisk" bumping down to the next line every time it is used.
We either have to have help with these issues, or get a refund and choose another add-on, which are relatively scarce for forms. I like most of the things about your form, but the column width issue, and the asterisk issue, and the words relating to the text fields being so far away from the text field itself is ruining it for us.
PLEASE respond, or we have to assume you are no longer providing support for your product, request a refund, and move on.

I wasn't able to respond, due to some days off.
The astrix is a css issue. Due to your theme css the astrix is "pulled" down.
You need to overwrite this in the template or view of formidable.
I'll respond to your other issues also.
