Hi,

3 suggestions to make your extension even better :

- create a "e-mail" field (that checks if it has the "aaa@bbb.ccc" form)

- create a "check mail" field (type it another time to check it fits with the first mention)

- possibility of sending the customer and / or the webmaster an e-mail that contains the answers to the form (as the extension "Extended form" does, for example), as an automatic confirmation.
