Form not working

We have tested our form over the past 2-weeks, adding in only a charge of .01 instead of the costs of $30 application fee.http://www.annapolisopera.org/vocal-competitions-applicant-informat... We launched the new pages today but now the form does not work...HELP PLEASE so my form works! My client is furious because the vocal competitions signup is limited in time! Hi, sorry that your experiencing difficulties with our add-on. Was it working fine when you had it set to $0.01? If so, was the price the only thing that you changed? Or did you create a new block/form?
Actually, it stopped working BEFORE I changed the price from .01 to $30. Should I try to recreate the block? Is it possible that it became corrupted? Has anything been updated to the ADD-ON? My client is really upset...attached are screen captures of the block...PLEASE - I'm trying NOT to lose a client...410-456-5281. NOTE: I re-created the block and it STILL does not work! Hi John, after looking at the PDF you sent me, I would suggest removing the field that says "Yes. I wish to pay the $30 application fee. [30]". There might be a problem since it takes up two lines; it might be confusing the add-on (I know, that would be a bug). Also, the Base Price is ADDED onto the cost of any options. So, with the way your form is now, it would charge customers $60. That is a second reason why you should remove the aforementioned field. You can add a similar field that asks for confirmation about the application fee; just don't include the [30] in it. Sorry, and let me know.
