editing mailchimp block

Hi,
we created a folder called 'templates' and copied the form.php in to the folder.
We copied from.php and renamed the file to 'newsletter.php' and 'blogsignup.php'
No modifying the HTML within this we can alter the text entry box titles and the submit button text.
The only other way we see being able to add a custom mailchimp signup for, e.g. one with firstname, lastname, age, country, submit is to create it athttp://www.mailchimp.com but then you cannot copy the embed code. Mailchimp only provide a way to embed a really simple form.
Shall we just keep looking at the API or does anybody else had an alternative way to add a custom signup form easily to C5?
