I've installed this add-on, but there's no block type available in the list?

Hello,
thanks for the question and your screencast. To the right of the plus [+] icon where you open the block-menu there is a list-icon that will display the settings for the Opt-In addon.

Here is a demo of the PRO-version (http://optinpro.concreteconversion.com/5.7/) In the top left corner, you'll see a yellow box with a welcome text. Above that box is the button that will open the settings.

The addon is not dependent on a specific block on a page. That way you get a better work-flow as pop-ups can't be treated as attached to one page. Another reason is not to intervene with the version control of the page.
