Where does the Submit button send the form? using what (i.e. email)?

I don't see where the submit button sends the form.. hopefully it's an email address...

The add-on applies a custom template to the Concrete5 Form block. So the Form block will be where you add an email address for the form submission. When in Edit Mode, if you click the block and choose Edit for the Form block you and then click on the Options tab you will see a place where you can add a email address for where the form submission email will be sent.
