the template is not showing either under settings as in your screenshot nor under the custom template button where the standard form template is to be found.

it seems the download version supplied is 9.3 instead of 9.4. How do I access the current version?

The purchased and installed version is 0.9.3.

Is this the reason why I can't access the template?
I am adding the standard form block, then accessing custom design & templates for that block.
Here I can choose the standard form template only.

Are you using this with the legacy form block or the standard form block? If it is the Standard form block then you can just use the Placeholders that come with that block in the newer version of version 8. If it is for the early version of 8 or the legacy block you can apply the custom template as shown in our video.

I am using the standard form block.
the concrete5 Version is 8.5.5
Placeholders are only available for text fields, not text areas or email or telephone number fields.

I have not heard back from you yet as to the package version you sent via message that I can't install.
Is therer someone looking into this? I urgently need to go online, and it this can't be resolved soon, I will unfortunately need to request a refund.
