Front End Form and Admin Approval

I just purchased this and and need help with a few items. First, the description mentions an optional "front-end" form which allows users to submit testimonials without having to go through the back end. I can't locate this form. Also, where on the back end does one go to actually approve/deny a testimonial submission. Finally, how do I display the testimonials that have been submitted and approved? Thanks in advance for response.

To create a testimonials in the backend, go to the "Testimonial" page in the dashboard and there will be an "Add New Testimonial" button. There will also be a list of testimonials with either an approve or unapprove link next to each one.
To display ones that are added and approved, add a "Studio Testimonials" block to any area of your site, and choose the appropriate options.
If you want to display the front end form, you need to check the "Show Submit Link" checkbox and then enter the text of what you want the link to show in the text box. Once you do this a link will show up by your testimonials, when you click on it a popup will show with a form the user can fill out.
