Add New Rows and Columns Buttons Not Working

When I create a new table and am on the Table Designer page, the add new column and row buttons are not creating new rows and columns, and I am unable to create a new table.

As I see there are errors on the console. The add-on assets (js & CSS files) are not loading:

/packages/whale_responsive_tables/css/app.css
/packages/whale_responsive_tables/js/app.bundle.js
/packages/whale_responsive_tables/js/custom-scrollbar/jquery.mCustomScrollbar.concat.min.js

Also, I noticed on 'Add Functionality' page, that the package icon is missing.

You can check on your server if those files exist and have the right permission.
